By entering my password and logging into the Neurocare Sleep Management Platform, I attest and agree that a) I will protect the privacy/confidentiality of the patients. protected health information ("PHI") contained within the systems, b) system use may be monitored, c) I will only access PHI contained in the system which I am authorized to access to carry out treatment, payment, and health care operations with respect to patients with which I or the practice has a treatment relationship, d) I will comply with Neurocare access policies summarized in the Neurocare Systems Access Confidentiality and Security Agreement and e) violation of Neurocare.s policies intended to protect PHI and to protect the security of the systems may result in sanctions, up to and including suspension, loss of access privileges, termination of employment or service contract, and/or criminal prosecution, in accordance with Neurocare.s policies.
